Our experienced team of physicians and licensed clinicians works one-on-one with survivors and care partners to identify recovery needs and deliver customized support through Principal Illness Navigation (PIN) and a stroke-specific telehealth app.

Our multi-disciplinary, holistic approach is grounded in best clinical practices for stroke survivors and provides a wide variety of personalized support. This includes coordinating needed follow-up appointments and rehabilitation services, educating stroke survivors on personal risk factors and medication management, addressing social determinants of health, and much more.

Once the initial consultation is complete, eligible patients are prescribed PIN and connected to a Kandu Navigator to implement a personalized Care Plan developed by the prescribing doctor.

The navigator provides resources and strategies to help them actively manage their recovery. The navigator is accessible via the Kandu App, allowing stroke survivors and their care partners to receive ongoing assistance and support remotely.

Kandu also provides access to a community of stroke survivors. These small group settings allow survivors to connect with others who are at a similar stage of their recovery.
