Kandu provides post-discharge support to stroke survivors and their care partners that aims to increase quality of life, create durability of clinical outcomes, and decrease readmissions and healthcare expenses.
Our experienced team of physicians and licensed clinicians works one-on-one with survivors and care partners to identify recovery needs and deliver customized support through Principal Illness Navigation (PIN) and a stroke-specific telehealth app.
Our multi-disciplinary, holistic approach is grounded in best clinical practices for stroke survivors and provides a wide variety of personalized support. This includes coordinating needed follow-up appointments and rehabilitation services, educating stroke survivors on personal risk factors and medication management, addressing social determinants of health, and much more.
Stroke survivors begin by meeting with a Kandu Physician for a virtual Evaluation and Management (E/M) visit.
Once the initial consultation is complete, eligible patients are prescribed PIN and connected to a Kandu Navigator to implement a personalized Care Plan developed by the prescribing doctor.
The navigator provides resources and strategies to help them actively manage their recovery. The navigator is accessible via the Kandu App, allowing stroke survivors and their care partners to receive ongoing assistance and support remotely.
Kandu also provides access to a community of stroke survivors. These small group settings allow survivors to connect with others who are at a similar stage of their recovery.
“Kandu allows us to continue to provide comprehensive care to patients after they leave our hospital. They are an important resource for patients when they need it most– during those first weeks and months after hospital discharge. Kandu’s focus on the whole person ensures patients are set up for success.”
Patrick Topping, MBA, BSN, RN
Stroke Program Coordinator at Cooper Neurological Institute
“Including Kandu into our program has really allowed us to have that full continuity of care and follow our patients for a longer period of time– to ensure they are being seen by their primary care and taking the proper medication, to prevent future strokes and future hospitalizations.”
Amanda Webb, MSN, RN, SCRN, NE-BC
Regional Director, Neuroscience Service Line at Christus Spohn Health System
